Use this form to propose your own PACE activity (i.e. internship / placement) as a part of your PACE unit.  Complete the form with input from your host supervisor to ensure the information is complete and correct. Information collected in this form will be stored securely and only used or released in accordance with the University’s Privacy Management Plan. You will be able to ‘Save & Exit’ the form at any time and you will receive the form link in your nominated email. Please do not forget to return to the form and ‘Submit’ it.

 

The process for a proposal involves several steps:

 

1. You (the student) complete and submit this form  

2. The PACE team will check the details provided by you and if happy return the form to you.  

3. You will review the advice of the PACE Team and submit the form to the person who has offered to be your Host Supervisor.   

4. Your Host Supervisor will:   

a. review information you submitted and amend/add where necessary, 

b. complete some WHS information and return the form to you. 

5. Once you have received the form back from the Host Supervisor you must:   

a. ensure all the information is complete and correct,   

b. complete a declaration and return the form to the PACE Team so that the Unit Convenor can assess the activity 

6. PACE staff will assess the information to determine whether the proposed activity meets the learning outcomes of your PACE unit, and whether the identified risks, and the proposed risk management strategies, will ensure your safety during the proposed activity.

 

Important: You cannot start your PACE activity until you have received formal confirmation from the PACE team that your activity has been approved and you have met any required background checks.  You will not be covered by the University’s insurance policy if you do not comply with this.
